Where does the phrase "truly, madly, deeply" originally come from?

Where does the phrase "truly, madly, deeply" originally come from?





It's not from the Cascada/Savage Garden song because several years earlier there was a movie with the same name starring Alan Rickman.





More than likely the title came from somewhere earlier. I read somewhere that it might be a poem or by a famous poet...





If you could be specific as to exactly where the phrase comes from, what poem or play or story, etc...
